24hr Supported Living - Mental Health
We offer support for service users with complex Mental Health needs age 18+. Our Service Manager Agnieszka, a qualified AMHP and BIA for DoLS, has 11 years of experience in community mental health services (this includes outreach and forensic mental health services), and is involved in the running and management of daily care to ensure quality risk management and support. Agnieszka's professional knowledge and extensive experience in mental health services as a Social Worker and Practice Manager contributes to providing a very reliable service to our clients.
Our team members are trained in supporting service users with learning disabilities and/or autism. They are determined to improve our clients’ health and wellbeing, and focused on achieving positive outcomes. Displaying challenging behaviour is a way of letting people know your feelings and emotions when you don’t have the communication or social skills to do so in other ways. When practicing positive behaviour support, we start by looking at the reasons behind each individual person’s behaviour challenging. We work closely with Social Services to ensure a safe home environment for all residents.
The service aims to reduce the occurrence of delayed hospital discharges and alleviate bed pressures. Under D2A service we assess and accommodate service users on the same day referral is received. Our Homes offer 24hr Supported Living for people with complex mental health difficulties and learning disabilities. We take Safeguarding procedures very seriously and work within the legislation and guidance provided by the Care Act 2014.
Our Homes are in a close proximity to transport and provide double and single bedrooms, all furnished with a mix of ensuite and shared bathroom/toilets facilities, a large living area, a kitchen-dinner and a large back garden. We have ample parking space that can accommodate four cars, for easy visitors’ access.
Our dedicated, experienced and committed team, offers services to hospitals to help people step down from inpatient wards, and to prevent readmission. We understand how vulnerable and fragile one can feel, when discharged back to the community following, at times, a lengthy hospital admission. We have a diligent assessment process that allows us to understand people’s needs, these include both the eligible under Care Act 2014 needs, as well as own cultural, religious and personal preferences. We offer assistance for people to feel safe and comfortable to return to living in the community. Our service manager, being a qualified Approved Mental Health Professional, provides guidance to risk assessments and overlooks care of patients discharged under Supervised Community Treatment. We work closely with mental health inpatient teams and mental health community services,
